
Eligibility:
1. Must have good dental hygiene practices
2. Must be enrolled in school with a C average or above GPA
3. Must have a social impediment or moderate/severe need for braces
4. Applicants are children or young adults (age can vary per provider)
5. This program helps people with income at or below 200% of federal poverty guidelines
6. Should demonstrate a willingness to get involved in the community through volunteer service
7. Some orthodontists may require 20 hours of volunteer service.
8. Must have positive letters of recommendation from at least two community leaders or teachers.
Smile for a Lifetime (S4L) is an international program that provides orthodontic scholarships to children
and young adults who normally would not be able to afford treatment. Over 200 providers throughout the
U.S. and Canada. S4L will not charge a fee for the orthodontic treatment –participating orthodontists and orthodontic manufacturing companies donate their service, including the cost of one set of retainers. The
only fees incurred are an initial $20 non-refundable application fee and a $480 investment fee to cover that
of the program.
