We are excited to offer a new program – Veteran Directed Care (VDC), a self-directed home care support program for Veterans.
With VDC, the Veteran is approved for an annual budget that can be used to purchase personal care assistance and specific goods that help them avoid nursing facility placement. No funds are directly paid
to the Vet; instead, they work with DNMM to determine what services will be purchased, such as
Personal care needs
Goods/services to assist in independent living
Care coordination
