As you get older, you deserve more comprehensive care. If you’re age 55 or older with chronic health conditions or disabilities, PACE Southeast Michigan will help you stay one step ahead.

PACE offers a unique line-up of coordinated care to help you stay in your home and community longer.

You can benefit from medical services like primary care, dental, vision, physical therapy, and more in our day centers, filled with caring staff and social activities. We’ll even pick you up and drop you off at the end of the day. Or we can serve you in the comfort of your home.

PACE Southeast Michigan provides care and services in the home, the community and our day health center.

• You are 55 years old or older
• Live in the PACE SE MI service area which includes all of Macomb County and most of Wayne & Oakland Counties
• Present characteristics qualifying you for nursing home level of care, as certified by the State of MI
• You are able to live safely in the community at the time of enrollment with support from PACE SE MI
