SERVICE AREA: Primarily Wayne County and Downriver communities, with some programs serving broader areas of Southeast Michigan or statewide. Specific programs may have their own service areas and eligibility requirements.

CLIENTS SERVED: Southeast Michigan residents. Some programs specifically serve older adults, adults with disabilities, pregnant women and parents, job seekers, nursing-facility residents, and WCCCD students.

DESCRIPTION: The Information Center is a nonprofit information, referral, advocacy, and support organization that connects people with public benefits, community services, and other resources. Clients can contact The Information Center for personalized assistance identifying resources for their specific situation. Services are free and confidential.

PROGRAMS/SERVICES:

• Information & Referral: Personalized referrals for housing and shelter, food, utility assistance, employment and training, clothing and household items, healthcare, mental health, substance use, legal services, home care, and more.
• MI Choice Medicaid Waiver: Helps eligible older adults and adults with disabilities obtain in-home and community-based services so they can avoid or delay nursing-facility placement. Includes personal care, home modifications, transportation, meals, respite, and medical supplies.
• Community Transition Services: Helps eligible people leave nursing facilities and return to community living. Assistance is provided with housing, furnishings, home setup, personal care, transportation, and necessary home modifications.
• Michigan Works! One Call: Connects job seekers with career services, training, employment opportunities, and resources to overcome employment barriers.
• 321-GROW Parent & Family Resource Line: Connects pregnant women and parents with young children to nurses, social workers, peer coaches, dietitians, home-visiting services, and community resources.
• Student Assistance Program: Helps Wayne County Community College District students overcome barriers to completing their education by connecting them with essential community resources.
• MI Options / Person-Centered Options Counseling: Free, unbiased counseling to help Michigan residents understand long-term-care choices, including home care, assisted living, and nursing-home options. The program serves the Lower Peninsula.
• Resource Database: Online searchable database and resource guide for locating community services and assistance.
