DUCHENNE FAMILY ASSISTANCE PROGRAM / Team Jospeh 

The Duchenne Family Assistance Program provides support related to the care of a child with Duchenne in order to ensure the best care and to ease the strain on families.

Please note: Members must have Duchenne Muscular Dystrophy.

This program provides:

– Financial assistance
– Help paying for healthcare

Assistance will include, but is not limited to:

– Equipment (scooters, manual wheelchairs, shower chairs, etc.)
– Accessible ramps
– Transportation to clinic visits (fares for car/van service, train fare, gas cards)
– Vehicle modifications
– Home modifications

Documents Required:
Letter of medical necessity, insurance denial, appeals, prescriptions, quotes and estimates, personal letter of need, other.
