What are the requirements to apply for SDMAP?

Pet owners must be facing and able to provide proof of one of the following: Government assistance (disability, SSI, SNAP, Medicaid, EBT) / Loss of employment (termination notice) or currently receiving unemployment benefits  / Income is less than or equal to 175% of the Federal Poverty Line  /            Eviction notice, Section 8 housing approval, or currently unhoused /Pet owners must have a senior dog, 7 years or older. Estimated age is okay, but it must be reflected on their medical records or confirmed by your veterinarian.

Senior dogs must have a pre-existing chronic medical condition that requires long-term medication. Some examples include, but are not limited to:  Diabetes / Cushing’s disease / Hypothyroidism / Heart disease / Osteoarthritis

Only prescription medication and prescription food are covered under the grant. We cannot provide over the counter supplements, supplies, or diagnostics. We cannot provide antibiotics, anti-anxiety medication, or any controlled substances under the grant. Exceptions may be made on a case-by-case basis.

Funding under SDMAP will be paid directly to the pet owner’s veterinarian. We are unable to have medication and food shipped from an online pharmacy, though we will let clients know on a case-by-case basis.
