MICHIGAN DENTAL PROGRAM (for HIV patients)
The Michigan Dental Program (MDP) covers dental care for people with HIV who qualify for the program. To be eligible for MDP, applicants must meet the following criteria:
Provide proof of HIV status / Be a resident of the State of Michigan / Have a gross income of less than 500% of the Federal Poverty Level (FPL). Income is evaluated based on the current FPL guidelines when the completed application is received. Income includes earned and/or unearned income (e.g. employment income, self-employment income, Supplemental Security Income).
In most cases, applicants must apply for public assistance (Medicaid and/or Healthy Michigan Plan) within the past 90 days and have a pending, denial or spend-down status. In all cases, MDP is payer of last resort. NOTE: If you pay for private dental insurance-through the Affordable Care Act, an employer or covered under a spouse-you are not eligible for MDP.
Covered dental services are based on pre-approved care plans and include both preventive and restorative care. Approved applicants will receive a letter from MDHHS. Delta Dental member cards will be mailed directly from Delta Dental to new members only.
