You must fill out a DTE EEA 2021/2022 Application for a refrigerator. This application can-
not be completed online, it must be printed out and completed. Keep in mind that when
this application is emailed out, it is intended for the email recipient(s) only (unless other-
wise noted) and it is anticipated that the instillation of approved appliance applications
will be scheduled as quickly as possible upon approval from DTE.
1. Valid ID
2. Current DTE Bill (ALL pages front & back. Plz make sure we can see name & Acct No.)
3. Current Proof of household Income/Benefits Letter
4. Proof of Ownership (Certified copy of Register of Deed,
5. Tax bill/statement in applicates name, Wayne County
6. Treasurer Statement, etc.)
Applications will only be accepted if all required documentation are attached! Upon com- pletion, clients can forward it back to Stephanie Williams via email, fax or drop off a hard copy. We will accept applications until the funding runs out! Availability is on a first come-first serve basis.
