The following Program of All -Inclusive Care for the Elderly (PACE) providers coordinate
care through Medicare and Medicaid and provide medical and social services to frail,
elderly individuals that generally allows them to continue living in their own community.

The focus of PACE Southeast Michigan is to help you live in the community and stay in your own residence for as long as possible. To meet this goal, PACE organizations focus
on preventive care. Although all people who enroll in PACE are eligible for nursing home
care, only 7 percent nationally live in nursing homes.
PACE services are designed to meet your individual needs:
PACE Southeast Michigan provides care and services in the home, the community and
our day health center.
1. You are 55 years old or older
2. Live in the PACE SE MI service area which includes all of Macomb County
and most of Wayne & Oakland Counties.
3. Present characteristics qualifying you for nursing home level of care, as
certified by the State of MI
4. You are able to live safely in the community at the time of enrollment with
support from PACE SE MI
