Team Wellness Center’s housing program is a block grant-based program that assists
members in moving toward self-sufficiency & living independently. Our housing specialists and Peer Support Specialists assist the member in finding and maintaining their
own residence by providing a structured setting and a plan for the member to follow.
The member will be provided with around the clock support when needed to ensure
their success in this life changing transition.
