Provides nutrition assessment, education and counseling, food assistance and referral services for eligible women and children up to age 5. Home visiting program for pregnant or new parents of children less than 3 months to provide social support, home visits, information and care coordination to promote healthy pregnancies, good birth outcomes and healthy infant development. Provides health insurance coverage for children with physical disabilities from birth to 21 years
old or individuals with cystic fibrosis or hemophilia covered beyond age 21.
