Senior Health Clinic: Our board-certified geriatric provider offers a senior health clinic every Wednesday (9:30am-12:30pm) with both in-person and telehealth services. Health screenings offered during these appointments include dementia, anxiety, depression, cancer, etc.
Insurance Navigation & Care Coordination: Our team of care managers and insurance navigators can assist patients with Medicare insurance navigation and the coordination of additional resources (i.e. transportation, housing, support groups).
Care Management: Care Management services are available at Corktown Health to assist community members and patients with obtaining physical, mental, and social services. Our team of trained Case Managers and Early Intervention Services Specialists connect individuals living with HIV to support services to ensure they are linked and remain in HIV medical care. The Care Management team also assists individuals not living with HIV by providing them with referrals and resources.
Food Voucher Program: Our Food Voucher program offers a $50 bi-monthly food voucher for clients living with HIV who meet the following program eligibility. Must be living with HIV / Income must be 150% below the Federal Poverty Level / Must be a resident of the Detroit Eligible Area (Wayne, Oakland, Macomb, St. Clair, Lapeer and Monroe.
Nutritional Supplement Program: Provides supplements such as Ensure, Glucerna, Nepro, for clients living with HIV in the Detroit Metropolitan Area with an “underweight” Body Mass Index (BMI) or who have experienced unplanned weight loss.
Medical Nutrition Therapy: Participation in Medical Nutrition Therapy with the Registered Dietitian is required when receiving food voucher and nutritional supplement services.
Behavioral Health: BHS provides weekly, out-patient therapy services to clients with mild to-moderate mental health concerns, including anxiety, depression, grief and loss, exploration of sexual orientation/gender/other identities, coping with HIV and other chronic illnesses, trauma survivorship, and more. Assessments and referrals to other local services are available for clients with more intensive needs, such a substance cessation, crisis services, and treatment for severe and persistent mental illnesses. All of our therapy services are evidence-based and may include traditional insight-based “talk therapy”, behavioral modalities such as Cognitive Behavioral Therapy, client education, socialization and supportive groups, as well as the inclusion of mental health related activities that support the mind-body connection, such as meditation and exercise. BHS is proud to partner with local universities, including Wayne State University and The University of Michigan to train future clinicians.
Oral Health for All: Corktown Health’s Dental Clinic provides inclusive, high-quality oral health care focused on prevention, mainten- ance, and treatment of disease. We serve adults, children, and patients with disabilities in a safe, affirming environment. Our staff are trained in trauma-informed, inclusive, and disability-aware care, ensuring every patient’s unique needs are respected. We also prioritize the oral health of people living with HIV, recognizing its impact on treatment and quality of life. By emphasizing prevention and early intervention, we aim to shift perceptions and promote lifelong oral wellness.

Offering Financial Support To Those Living With HIV:
The Housing and EFA programs provide emergency financial assistance to households experiencing utility shutoff (past due bills), eviction, need for assistance with first month’s rent for a new move.
This type of assistance is designed to help clients living with HIV stay engaged or to reengage in medical care. When an applicant does not meet the criteria of our programs, we work with our network to connect the client to additional resources.
Qualifications: Persons Must be living with HIV / Must be a resident of the Detroit Metropolitan Area / Wayne, Oakland, Macomb, St. Clair, Lapeer, and Monroe counties / Income must be 250% below the Federal Poverty Level / Must have an identified emergency / Eviction notice, past due/outstanding bills, invoice for first month’s rent, etc.
HIV/STI Prevention Services at Corktown Health: On-site HIV/STI Testing: Services include HIV/STI screening, testing and treatment for patients and their partners. Insurance Navigation: Assistance with health insurance and medication assistance programs to ensure access to prevention and treatment medications. PrEP and PrEP Navigation: Obtain and maintain prescriptions and related health care, as well as identifying and reducing barriers to care. PEP: Offering medicine to prevent HIV within 72 hours of a possible exposure. On-site Safer Sex, hygiene and menstrual kits / Condoms, pads, toothbrushes, razors, tampons and more available on-site at Corktown Health.
