Who is eligible for Mom’s Meals?
As a leading provider of medically tailored, home-delivered meals in the U.S., we work with a wide variety of health plans and organizations. We also serve individuals and caregivers who order meals on their own.(See specialty meals below)
Medicaid and Older American Act Recipients:
We offer meals to recipients under certain Medicaid and Older American Act. Eligibility for meals varies
by state and by program. If you have a Case Manager, he or she will be able to tell you if home-delivered
meals are part of your benefits. If you don’t know or don’t have an assigned Case Manager, you can get
in touch with your local Area on Aging, who may be able to help.
Medicare Advantage Beneficiaries:
Many Medicare Advantage plans offer meals as part of their post-discharge or chronic care support programs. To see if yours does, consult you plan materials or call the phone number on the back of your insurance card.
Self-pay Individuals:
We offer a self-pay program for those without an eligible health plan or who don’t qualify for government assistance but still want the convenience of home-delivered meals for themselves or a loved one.
Mom’s Meals® offers halal-certified meals through a partnership with Rajbhog Foods as part of our comprehensive food as medicine solutions. Through a partnership with Amy’s Kitchen, Mom’s Meals® offers kosher meal delivery that individuals can access as part of their health plan benefit. Certified by Tablet-K, all kosher meals are vegetarian – some may contain dairy.
